Application for Automobile Insurance in Ontario – Owner’s Form (OAF1)

When you first apply for car insurance in Ontario you will need to complete an application form, known as the Ontario Application for Automobile Insurance (OAF1). If a licensed broker or agent fills out the form then make sure to check that it has been filled out accurately.

Another good practice is to research the name of person that is providing you with car insurance to ensure they are legally approved to sell you insurance. You can do a search for agents licensed in Ontario on the Financial Services Commission of Ontario (FSCO) website.
